Dental coverage inside a large health plan
UnitedHealthcare is best known as a medical insurer. Dental may be UnitedHealthcare Dental, a stand-alone PPO, or a limited dental benefit on a Medicare Advantage plan. Each one has its own ID details. Billing a cleaning to the medical payer is a common reason claims deny, so we separate the cards at the desk.
Dentality Family Dentistry accepts UnitedHealthcare dental plans and files the claim. For an employer PPO we verify the maximum, deductible, and coinsurance. For a Medicare dental benefit we verify the allowance and the covered procedures. We tell you which of those you have before we recommend a full set of treatment.
UnitedHealthcare medical and UnitedHealthcare dental are easy to mix up because the brand is the same. A cleaning is never billed to the medical payer. If dental is included inside a health plan, it still has dental rules, a dental ID, and often a separate allowance.

What changes the estimate
Frequency limits, missing-tooth clauses, and alternate-benefit downgrades change what UnitedHealthcare pays even when the plan is active. A plan can be "accepted" and still leave a crown mostly unpaid because the annual maximum is already gone. That is not a surprise we want you to have later, so the verification happens first.
Out-of-network language on a UnitedHealthcare dental PPO means the plan may still pay something if this office is not in a particular network tier, at a lower level. We will say which situation your card creates. We will not describe every UnitedHealthcare network as in-network.
Medicare Advantage dental benefits under UnitedHealthcare are frequently a yearly allowance rather than a full PPO. We check the remaining amount and the procedure list so a visit is not planned as if a commercial maximum applied.

What to bring
Out-of-network wording on a UnitedHealthcare dental PPO means the plan may still pay something, at a lower level. The estimate will say whether we are billing an in-network benefit or an out-of-network one. We do not describe every UnitedHealthcare network as in-network.

- The UnitedHealthcare dental card, separate from the medical card when you have both
- Member ID and group number
- Any Medicare plan letter that lists a dental allowance
- Dates of dental claims already filed this benefit year
How we bill
We submit the dental claim to the payer on the dental card and respond to requests for records. You get the explanation of benefits through your UnitedHealthcare account. We match it to the estimate.
If the plan pays little or nothing toward the care you want, we will show the office fee and the financing options before you decide. The full list of accepted plans is on the insurance and financing page.
If you have already filed dental claims this year through a UnitedHealthcare account, the dates help us see frequency limits. An app screenshot of recent dental claims is as useful as the card.
