United Concordia Dental Insurance in Katy

United Concordia administers employer dental plans and some military and federal dental programs. The program name on your card decides how we bill. The sections below explain what to bring, how this office files or quotes the visit, and which details on the card change your estimate.

More than one United Concordia program

United Concordia is a dental carrier. Many patients have it through an employer PPO. Others have it because United Concordia administers a military dental program, including dental coverage for eligible family members and retirees, or a federal dental option. Those programs have their own handbooks. A commercial PPO claim and a military dental claim are not interchangeable forms.

Dentality Family Dentistry accepts United Concordia. We verify the program before the visit. If your card is a military or federal dental program, we confirm that this office can bill that program. We will not describe every United Concordia card as the same network, and we will not start treatment that the program will not process.

Employer United Concordia PPO cards and military or federal dental cards are administered by the same company and processed as different programs. We ask which one you have before we book major treatment, because the covered-service list is not the same.

What a commercial PPO covers

On an employer PPO, expect the usual three classes of service. Preventive care is exams, cleanings, and routine X-rays. Basic care includes fillings. Major care includes crowns and bridges. Annual maximums, deductibles, and waiting periods come from your group. Implants and orthodontics are checked separately.

Military and federal dental programs use their own covered-service lists and cost shares. If you are used to a civilian PPO, do not assume the same percentages. We read the program you actually have.

A commercial PPO still uses preventive, basic, and major classes, with a group maximum and sometimes a waiting period. A military dental program uses its own cost shares. If you are used to a civilian plan, do not assume those percentages carry over.

What to bring

Sponsor information matters on a family or military policy. The sponsor's ID, not only the patient's name, is often what United Concordia uses to find the contract. Bring that number if it is printed on the card.

  • Sponsor or subscriber information on a family or military policy
  • Any other dental plan, so benefits can be coordinated
  • Dates of treatment already paid this benefit year

Claims

We file the United Concordia claim for the program on the card and send X-rays if they are requested. You receive the explanation of benefits. We go over your estimated portion before treatment, including when a program pays less than a commercial PPO would.

Every plan we accept is listed on the insurance and financing page.

We will not start a crown on the assumption that every United Concordia program pays major care the same way. Call with the member ID and we will read the program you actually have, including any waiting period still in effect.
