How Lincoln Financial dental is set up
Lincoln Financial Group dental benefits are typically employer PPOs. The human-resources booklet and the wallet card can use slightly different plan names. The card is what we bill. A life or disability card from Lincoln is not a dental card, and we cannot file a cleaning to it.
Dentality Family Dentistry accepts Lincoln Financial dental insurance and submits the claim. We verify eligibility, the deductible, the annual maximum, and whether you are still inside a waiting period. Waiting periods are common on new hires for basic or major services even when preventive care is available immediately.
New employees are the group we slow down for. Lincoln Financial often pays preventive care immediately and holds major care behind a waiting period. We check that date before we prepare a tooth for a crown.

Preventive, basic, and major care
Your group decides the coinsurance. In general, dental PPOs from Lincoln and similar carriers pay the largest share of exams and cleanings, a middle share of fillings, and a smaller share of crowns, bridges, and dentures. Implants and orthodontics need their own lookup. We will tell you if the booklet excludes them rather than let a denied claim be the way you find out.
If you already had a pretreatment estimate from Lincoln for work that was never started, bring it. Those estimates expire, and a new one may be wiser than relying on a letter from last year.
A life-insurance or disability card from Lincoln is not dental coverage. If the only Lincoln card you have does not say dental, call human resources for the dental ID before the visit so we are not billing the wrong plan.

What to bring
Pretreatment estimates from Lincoln expire. If you have a letter from last year for work that was never started, bring it and we will tell you whether it is still usable or whether we should send a new one.

- The Lincoln Financial dental ID card
- The employer group number
- Subscriber name and date of birth for dependent coverage
- Any pretreatment estimate still in its valid window
After the visit
We file electronically and answer requests for X-rays or notes. You receive Lincoln's explanation of benefits. If the payment does not match the estimate, we walk through the reason — a frequency limit, a downgrade to an alternate filling material, or a used-up maximum are the usual ones.
Larger balances can be financed. Every carrier on our list, including Lincoln Financial, is linked from the insurance and financing page.
Downgrades and frequency limits are the usual reason a Lincoln payment is smaller than the estimate. We will match the explanation of benefits to the claim we sent and explain the line that changed.
