Two kinds of Humana cards
An employer Humana dental PPO behaves like other dental PPOs. There is a deductible, an annual maximum, and different coinsurance for preventive, basic, and major care. We file a claim and Humana pays its share.
A Humana Medicare Advantage card is different. Some of those plans include a dental allowance — a yearly dollar amount for certain dental services — rather than a full PPO. The allowance, the list of covered codes, and whether the dentist must be in a Medicare dental network are all on that plan. Dentality Family Dentistry accepts Humana dental benefits and will tell you which kind you have before we schedule treatment that could exhaust a small allowance in one visit.
Please say whether the Humana card is from an employer or from a Medicare plan. An employer PPO is filed as dental insurance. A Medicare dental allowance is a yearly dollar amount with its own list of covered procedures, and a large crown can use the entire allowance in one visit.

What we verify
For a PPO we check the remaining maximum, deductible, waiting periods, and frequency limits. For a Medicare dental allowance we check the remaining dollars, which procedures count, and whether a claim or an office payment against the allowance is the right path. We will not spend a $1,000-style allowance on a procedure the plan excludes and then surprise you.
Humana dental HMOs, when the card assigns a primary dentist, are not the same as the PPO. If your card names a facility, call us before the visit.
We will not schedule a full treatment plan against a small Medicare allowance without showing you the remaining dollars first. If the allowance covers the exam and not the crown, you hear that before we start.

What to bring
Humana dental HMO cards that name a primary dentist are not billed like the PPO. If a facility is printed on the card, read it to us on the phone so we can confirm this office is the one the plan allows.

- The Humana card that lists dental benefits
- A second card if medical and dental are separate
- Any Medicare plan documents that mention a dental allowance, if that is the benefit you mean to use
- Dates of dental treatment already billed to Humana this year
Billing
We file Humana dental claims when the plan is a PPO, and we follow the allowance rules when the plan is a Medicare dental benefit. Either way, you hear the expected cost before we start. If Humana requests records, we send them.
The rest of the plans we accept are on the insurance and financing page.
Bring any Humana letter that lists dental, even if the plastic card looks like a medical card. The letter often has the allowance and the included codes that the card itself leaves off.
