How an Ameritas dental plan works here
Ameritas sells dental benefits mostly through employers. The plan name on your card — often a PPO — matters more than the logo. Deductibles, annual maximums, and the split between preventive, basic, and major care are set by that group contract.
Dentality Family Dentistry accepts Ameritas and files the claim for you. We ask for the member ID before the visit when we can, so the estimate is ready when you arrive. That estimate is a preview, not a guarantee: Ameritas makes the final payment decision when it processes the claim.
Ameritas groups are written by employers, so a neighbor with Ameritas can have a different maximum and a different cleaning frequency than you do. We treat the group number as the plan, not the logo.

What patients usually want to know
Cleanings, exams, and routine films are the services dental PPOs, including most Ameritas groups, are built to encourage. Basic care such as fillings is next. Major care such as crowns, bridges, and dentures often has a lower coinsurance and sometimes a waiting period if you enrolled recently. Implants are the item we always check line by line, because some groups cover the crown on an implant and not the implant itself, or the reverse.
If you have already used part of the annual maximum at another office this year, tell us. Ameritas will not pay past that cap, and we would rather plan the work in the right order than discover the cap after the visit.
If Ameritas already paid for treatment this year at another office, that spending still counts. Tell us the month and the type of visit so we do not plan a crown that would land entirely on you because the maximum is already gone.

What to have ready
Dependents should bring the subscriber's date of birth as well as the card. Ameritas matches family members to the contract with that date, and a missing birth date is a common reason a first claim sits unpaid.

- Ameritas dental ID card, front and back
- Subscriber name and date of birth if you are a dependent
- The approximate date of your last cleaning, if it was not here
- Any pretreatment estimate Ameritas already sent you for pending work
Claims and follow-up
We submit Ameritas claims electronically and track them. If the carrier pend the claim for a missing X-ray or periodontal chart, our office sends it. You will still receive Ameritas's explanation of benefits, which is the document that shows what they allowed and what they paid.
A remaining balance can be paid at the visit or discussed as financing. Every plan we accept, including Ameritas, is listed on the insurance and financing page.
A pretreatment estimate is worth requesting when the visit is more than a cleaning. We will tell you how long Ameritas usually takes to answer and whether it is wiser to wait or to start with the portion you are comfortable paying.
